Iz Laboratorija za naprednu genomiku Instituta "Ruđer Bošković" potvrdili su danas da je u Hrvatskoj od ukupno 50 analiziranih uzoraka, u jednom uzorku iz Zagreba, uzetom 31. siječnja, potvrđena mutacija u proteinu šiljku na položaju E484K koji je utvrđen kod brazilskog i južnoafričkog soja koronavirusa. To, automatski ne znači da se u Hrvatskoj proširio brazilski soj jer genom virusa s tom mutacijom još nije sekvenciran i za sada se ne može reći je li riječ o južnoafričkom soju, brazilskom ili pak o nekom novom hrvatskom.

"Ta varijanta koju smo detektirali, prisutna je kod jednog pacijenta koji se ponovno zarazio koronom u roku od tri mjeseca. Zato je to pod povećalom, vidjet ćemo točno o čemu se radi. Virus stalno stječe nove mutacije, na nama je da ih pratimo - objasnio je dr. Oliver Vugrek, šef Laboratorija za naprednu genomiku.

Riječki imunolog, prof. dr. Zlatko Trobonjača kaže nam kako se brazilski soj širi brzo, kao i britanski jer ima mutaciju na 501 mjestu u proteinu šiljka koja mu to omogućava. Pozitivno je što nema dokaza da uzrokuje ozbiljnije kliničke slike pri razvoju bolesti.

„Brazilska inačica ima još desetak mutacija, a pored ove na 501 mjestu, važna je i mutacija na 484 mjestu u proteinu. Ona mu naime, u velikoj mjeri omogućuje izbjegavanje imunosnog odgovora organizma – objašnjava naš sugovornik.

Također naglašava kako je moguće da se osobe koje su već preboljele wuhanski soj koronavirusa, nanovo zaraze brazilskim sojem.

„Naravno da ne bi bila dobra vijest ako je stigao brazilski soj, što će se tek utvrditi, no budući već u organizmu osoba koje su prebolje COVID postoje antitijela, vjerojatno je da će većina njih ako se zaraze brazilskim sojem, odbolovati zarazu bez težih komplikacija. Izuzetno mali postotak mogao bi imati teže komplikacije, a vjerujem da bi smrtnost bila gotovo nula – napominje ugledni imunolog.  

Što se tiče cjepiva, kaže kako još nemamo dovoljno informacija kako ona djeluju na južnoafrički i brazilski soj.

„No, svako cjepivo spriječit će teže oblike bolesti, pa tako i u slučaju ovog soja, ako oboljeli neće imati potpunu zaštitu, cjepivo će ih štititi od težih oblika bolesti, hospitalizacije te dugotrajnog COVID-a, a to je u cijeloj priči najvažnije – smatra prof. Trobonjača.

Zanimljiva je priča o brazilskom soju virusa, budući se sve donedavno brazilski grad Manaus spominjao kao ogledni primjer postizanja 'imuniteta krda' nakon što je zaključeno da je u njemu, virusom iz Wuhana, zaraženo više od 70 posto stanovnika. Smatralo se da je postignuta razina zaraženosti koja je dovoljna za kolektivni imunitet. Procijenjeno je tako da je do konca srpnja, skoro 76 posto stanovnika Manausa preboljelo COVID-19. No zaraza je nanovo buknula u prosincu, a broj zaraženih se gotovo izravnao s onim iz ožujka i travnja.

Luka Čičin Šain, virusni imunolog u Centru za infektološka istraživanja Braunschweig u Njemačkoj u više je navrata naglasio kako ga brinu brazilski i južnoafrički soj jer je za njih jasno da postoje naznake da ih imunološka memorija stečena prethodnom infekcijom teže prepoznaje.

- U Brazilu se populacija u nekim regijama u Amazoni, već prirodno prokužila. Pojava novog soja bila je reakcija virusa na populaciju koja je imuna. Zato što se virus nije mogao razmnožavati, došlo je do mutacije. Postoji ogromna razlika između prirodne imunosti i one koju dobivamo cjepivom. Već postoje studije oko toga, efikasna cjepiva imaju toliku količinu protutijela koji izaziva takav titar, da je on dovoljan da iskontrolira i one viruse koji se razlikuju. Njihova kontrola nije savršena, ali je sasvim dobra ako u startu imamo dovoljno protutijela. To je razlog zašto se treba cijepiti i ne računati da prirodna prokuženost obavi isti taj posao, jer neće – komentirao je Luka Čičin Šain.

Beroš: Za tjedan dana znat ćemo kakav je soj: brazilski ili hrvatski.

Komentirajući otkriće ministar zdravstva Vili Beroš je rekao: “Stvari nisu tako jednostavne. Na Institutu Ruđer Bošković detektirana je jedna od mogućih mutacija koje karakteriziraju ne samo brazilski, nego i južnoafrički i britanski soj virusa. Čuo sam se s dr. Vugrekom, rekao je da je sekvenciranje cijelog genoma virusa u postupku i za tjedan dana ćemo imati kompletnu informaciju.”

Po ponašanju, brazilski je soj ekvivalent britanskom soju. No, jedna mutacija na tom šiljku nalazi se u svim sojevima. Zato je sad nemoguće reći je li to brazilski, britanski ili južnoafrički ili će biti neka hrvatska varijanta virusa”, dodao je ministar.

Objasnio je i kako svaka pojedina varijanta virusa ima specifične mutacije, ovo je jedna od njih. "S kliničkog aspekta ta je mutacija nađena u sklopu analize 50 uzoraka. S obzirom na vremenski odmak nema kliničkih parametara za ubrzano širenje pandemije. Tome treba posvetiti pažnju, ali bez senzacionalizma”, istaknuo je Beroš.

Iz Laboratorija za naprednu genomiku Instituta "Ruđer Bošković" potvrdili su danas da je u Hrvatskoj od ukupno 50 analiziranih uzoraka, u jednom uzorku iz Zagreba, uzetom 31. siječnja, potvrđena mutacija u proteinu šiljku na položaju E484K koji je utvrđen kod brazilskog i južnoafričkog soja koronavirusa. To, automatski ne znači da se u Hrvatskoj proširio brazilski soj jer genom virusa s tom mutacijom još nije sekvenciran i za sada se ne može reći je li riječ o južnoafričkom soju, brazilskom ili pak o nekom novom hrvatskom.

"Ta varijanta koju smo detektirali, prisutna je kod jednog pacijenta koji se ponovno zarazio koronom u roku od tri mjeseca. Zato je to pod povećalom, vidjet ćemo točno o čemu se radi. Virus stalno stječe nove mutacije, na nama je da ih pratimo - objasnio je dr. Oliver Vugrek, šef Laboratorija za naprednu genomiku.

Riječki imunolog, prof. dr. Zlatko Trobonjača kaže nam kako se brazilski soj širi brzo, kao i britanski jer ima mutaciju na 501 mjestu u proteinu šiljka koja mu to omogućava. Pozitivno je što nema dokaza da uzrokuje ozbiljnije kliničke slike pri razvoju bolesti.

„Brazilska inačica ima još desetak mutacija, a pored ove na 501 mjestu, važna je i mutacija na 484 mjestu u proteinu. Ona mu naime, u velikoj mjeri omogućuje izbjegavanje imunosnog odgovora organizma – objašnjava naš sugovornik.

Također naglašava kako je moguće da se osobe koje su već preboljele wuhanski soj koronavirusa, nanovo zaraze brazilskim sojem.

„Naravno da ne bi bila dobra vijest ako je stigao brazilski soj, što će se tek utvrditi, no budući već u organizmu osoba koje su prebolje COVID postoje antitijela, vjerojatno je da će većina njih ako se zaraze brazilskim sojem, odbolovati zarazu bez težih komplikacija. Izuzetno mali postotak mogao bi imati teže komplikacije, a vjerujem da bi smrtnost bila gotovo nula – napominje ugledni imunolog.  

Što se tiče cjepiva, kaže kako još nemamo dovoljno informacija kako ona djeluju na južnoafrički i brazilski soj.

„No, svako cjepivo spriječit će teže oblike bolesti, pa tako i u slučaju ovog soja, ako oboljeli neće imati potpunu zaštitu, cjepivo će ih štititi od težih oblika bolesti, hospitalizacije te dugotrajnog COVID-a, a to je u cijeloj priči najvažnije – smatra prof. Trobonjača.

Zanimljiva je priča o brazilskom soju virusa, budući se sve donedavno brazilski grad Manaus spominjao kao ogledni primjer postizanja 'imuniteta krda' nakon što je zaključeno da je u njemu, virusom iz Wuhana, zaraženo više od 70 posto stanovnika. Smatralo se da je postignuta razina zaraženosti koja je dovoljna za kolektivni imunitet. Procijenjeno je tako da je do konca srpnja, skoro 76 posto stanovnika Manausa preboljelo COVID-19. No zaraza je nanovo buknula u prosincu, a broj zaraženih se gotovo izravnao s onim iz ožujka i travnja.

Luka Čičin Šain, virusni imunolog u Centru za infektološka istraživanja Braunschweig u Njemačkoj u više je navrata naglasio kako ga brinu brazilski i južnoafrički soj jer je za njih jasno da postoje naznake da ih imunološka memorija stečena prethodnom infekcijom teže prepoznaje.

- U Brazilu se populacija u nekim regijama u Amazoni, već prirodno prokužila. Pojava novog soja bila je reakcija virusa na populaciju koja je imuna. Zato što se virus nije mogao razmnožavati, došlo je do mutacije. Postoji ogromna razlika između prirodne imunosti i one koju dobivamo cjepivom. Već postoje studije oko toga, efikasna cjepiva imaju toliku količinu protutijela koji izaziva takav titar, da je on dovoljan da iskontrolira i one viruse koji se razlikuju. Njihova kontrola nije savršena, ali je sasvim dobra ako u startu imamo dovoljno protutijela. To je razlog zašto se treba cijepiti i ne računati da prirodna prokuženost obavi isti taj posao, jer neće – komentirao je Luka Čičin Šain.

Beroš: Za tjedan dana znat ćemo kakav je soj: brazilski ili hrvatski.

Komentirajući otkriće ministar zdravstva Vili Beroš je rekao: “Stvari nisu tako jednostavne. Na Institutu Ruđer Bošković detektirana je jedna od mogućih mutacija koje karakteriziraju ne samo brazilski, nego i južnoafrički i britanski soj virusa. Čuo sam se s dr. Vugrekom, rekao je da je sekvenciranje cijelog genoma virusa u postupku i za tjedan dana ćemo imati kompletnu informaciju.”

Po ponašanju, brazilski je soj ekvivalent britanskom soju. No, jedna mutacija na tom šiljku nalazi se u svim sojevima. Zato je sad nemoguće reći je li to brazilski, britanski ili južnoafrički ili će biti neka hrvatska varijanta virusa”, dodao je ministar.

Objasnio je i kako svaka pojedina varijanta virusa ima specifične mutacije, ovo je jedna od njih. "S kliničkog aspekta ta je mutacija nađena u sklopu analize 50 uzoraka. S obzirom na vremenski odmak nema kliničkih parametara za ubrzano širenje pandemije. Tome treba posvetiti pažnju, ali bez senzacionalizma”, istaknuo je Beroš.
