Rodilja koja je u ponedjeljak umrla u varaždinskoj bolnici imala je teški oblik COVID-a, a dijete rođeno carskim rezom zasad je stabilno, no još se ne zna je li i ono pozitivno, izjavio je u utorak ravnatelj varaždinske bolnice Nenad Kudelić.

Kudelić je rekao da je u ponedjeljak u popodnevnim satima na objedinjeni hitni bolnički prijem stigla 31-godišnja trudnica u 32. tjednu trudnoće s kliničkom slikom akutne respiratorne bolesti te da je testiranjem i obradom utvrđeno da se radi o teškom obliku COVID-a, s teškom obostranom upalom pluća.

Carski rez

“Trudnica je odmah zaprimljena u jedinicu intenzivnog liječenja, a zbog pogoršanja zdravstvenog stanja, prvenstveno uzrokovanog teškom upalom pluća, donesena je odluka o hitnom carskom rezu i započeta je reanimacija, no unatoč svim postupcima, rodilja je preminula”, kazao je.

Porođeno je zdravo muško dijete težine 2200 grama, koje zasad nije životno ugroženo i ostat će u bolnici, a je li i ono zaraženo COVID-om znat će se nakon rezultata testiranja.

Kudelić je istaknuo kako to nije bila jedina trudnica s COVID-om u varaždinskoj bolnici.

“Naučili smo da koronavirus više ne bira, u posljednjih desetak dana radilo se o nekoliko trudnica. Jednu rodilju smo nakon desetak dana od poroda zaprimili u bolnicu i zbog kardijalnog aresta i reanimacije morali smo je premjestiti u KB Dubrava”, dodao je.

Zbog čega je i od čega točno preminula trudnica, utvrdit će se obdukcijom. Jasno je da je, nažalost, preminula uz COVID, a koji je stvarni uzrok smrti koji je bio u "podlozi" i koji se moguće nikada ne bi razvio da nije bilo COVID-a, trebat će pričekati riječ patologa.

No, o "blaženom stanju" u doba COVID-a razgovarali smo s poznatim i jednim od splitskim ženama omiljenijih ginekolega, koji radi u privatnoj praksi, a koji je želio ostati anoniman.

- Ne želim na tuđoj muci raditi sebi reklamu. To nije ljudski ni etički. No, mogu pojasniti o čemu bi se u ovom slučaju moglo raditi. Dakle, cijeli svijet danas govori o povezanosti tromboza i cjepiva protiv COVID-a. No, istovremeno nitko ne govori o povezanosti tromboza i trudnoće.

Trudnoća je sama po sebi izuzetno trombogeno stanje i rizik za dobivanje tromba u trudnoći je puno veći negoli u općoj populaciji. Možda ljudi o tome ne razmišljaju, ali trombozu će prije dobiti neka trudnica negoli ona žena koja uzima kontraceptive ili ona koja se cijepila. Cjepivo protiv COVID-a puno rjeđe izaziva trombozu negoli trudnoća. To još uvijek ne znači da je rizik za dobivanja tromba u trudnoći golem, ali je svakako prisutan - kaže naš sugovornik.

Ginekolozi su svjesni ovog problema.

- Takva stanja uglavnom se događaju kod nekih trudnica koje imaju gensku predispoziciju za dobivanje tromba ili onih koje su gojaznije, koje više leže u trudnoći ili čak onih koje puše tijekom trudnoće. Dakle, uzroci smrti mlade žene u Varaždinu još nam uvijek nisu poznati, ali nije isključeno da je dobila tromb zbog kojega je preminula. To je prvo što mi "pada na pamet" kada imamo ovakav slučaj - rekao nam je splitski ginekolog.

Heparinom protiv tromba

Za sprječavanje smrtnog ishoda liječnici bi trebali trudnicama kod kojih se pojavi tromb davati niskomolekularni heparin.

- Njega treba davati kao profilaksu (za sprječavanje). S obzirom na to da je COVID novo oboljenje i da iz dana u dan svi o njemu učimo, mišljenja sam da bi se svaka trudnica, ako dobije COVID, trebala, osim svom obiteljskom liječniku, javiti i svom ginekologu, koji će joj, što je prije moguće, uključiti u terapiju niskomolekularni heparin, kako bi spriječio moguće teške komplikacije - kaže naš sugovornik.

Ujedno, u posljednjih godinu dana otkako je COVID sveprisutan, svjetski su ginekolozi došli do jednog zapanjujućeg podatka.

- Primijetili smo da ima dosta više tromboza kod trudnica koje su COVID pozitivne. Nikada nije bilo toliko tromboza. Zbog toga su se na svjetskoj razini donijele smjernice za sprječavanje tromboze kod trudnica. I sve te preporuke idu prema niskomolekularnom heparinu - zaključio je naš sugovornik.

Rodilja koja je u ponedjeljak umrla u varaždinskoj bolnici imala je teški oblik COVID-a, a dijete rođeno carskim rezom zasad je stabilno, no još se ne zna je li i ono pozitivno, izjavio je u utorak ravnatelj varaždinske bolnice Nenad Kudelić.

Kudelić je rekao da je u ponedjeljak u popodnevnim satima na objedinjeni hitni bolnički prijem stigla 31-godišnja trudnica u 32. tjednu trudnoće s kliničkom slikom akutne respiratorne bolesti te da je testiranjem i obradom utvrđeno da se radi o teškom obliku COVID-a, s teškom obostranom upalom pluća.

Carski rez

“Trudnica je odmah zaprimljena u jedinicu intenzivnog liječenja, a zbog pogoršanja zdravstvenog stanja, prvenstveno uzrokovanog teškom upalom pluća, donesena je odluka o hitnom carskom rezu i započeta je reanimacija, no unatoč svim postupcima, rodilja je preminula”, kazao je.

Porođeno je zdravo muško dijete težine 2200 grama, koje zasad nije životno ugroženo i ostat će u bolnici, a je li i ono zaraženo COVID-om znat će se nakon rezultata testiranja.

Kudelić je istaknuo kako to nije bila jedina trudnica s COVID-om u varaždinskoj bolnici.

“Naučili smo da koronavirus više ne bira, u posljednjih desetak dana radilo se o nekoliko trudnica. Jednu rodilju smo nakon desetak dana od poroda zaprimili u bolnicu i zbog kardijalnog aresta i reanimacije morali smo je premjestiti u KB Dubrava”, dodao je.

Zbog čega je i od čega točno preminula trudnica, utvrdit će se obdukcijom. Jasno je da je, nažalost, preminula uz COVID, a koji je stvarni uzrok smrti koji je bio u "podlozi" i koji se moguće nikada ne bi razvio da nije bilo COVID-a, trebat će pričekati riječ patologa.

No, o "blaženom stanju" u doba COVID-a razgovarali smo s poznatim i jednim od splitskim ženama omiljenijih ginekolega, koji radi u privatnoj praksi, a koji je želio ostati anoniman.

- Ne želim na tuđoj muci raditi sebi reklamu. To nije ljudski ni etički. No, mogu pojasniti o čemu bi se u ovom slučaju moglo raditi. Dakle, cijeli svijet danas govori o povezanosti tromboza i cjepiva protiv COVID-a. No, istovremeno nitko ne govori o povezanosti tromboza i trudnoće.

Trudnoća je sama po sebi izuzetno trombogeno stanje i rizik za dobivanje tromba u trudnoći je puno veći negoli u općoj populaciji. Možda ljudi o tome ne razmišljaju, ali trombozu će prije dobiti neka trudnica negoli ona žena koja uzima kontraceptive ili ona koja se cijepila. Cjepivo protiv COVID-a puno rjeđe izaziva trombozu negoli trudnoća. To još uvijek ne znači da je rizik za dobivanja tromba u trudnoći golem, ali je svakako prisutan - kaže naš sugovornik.

Ginekolozi su svjesni ovog problema.

- Takva stanja uglavnom se događaju kod nekih trudnica koje imaju gensku predispoziciju za dobivanje tromba ili onih koje su gojaznije, koje više leže u trudnoći ili čak onih koje puše tijekom trudnoće. Dakle, uzroci smrti mlade žene u Varaždinu još nam uvijek nisu poznati, ali nije isključeno da je dobila tromb zbog kojega je preminula. To je prvo što mi "pada na pamet" kada imamo ovakav slučaj - rekao nam je splitski ginekolog.

Heparinom protiv tromba

Za sprječavanje smrtnog ishoda liječnici bi trebali trudnicama kod kojih se pojavi tromb davati niskomolekularni heparin.

- Njega treba davati kao profilaksu (za sprječavanje). S obzirom na to da je COVID novo oboljenje i da iz dana u dan svi o njemu učimo, mišljenja sam da bi se svaka trudnica, ako dobije COVID, trebala, osim svom obiteljskom liječniku, javiti i svom ginekologu, koji će joj, što je prije moguće, uključiti u terapiju niskomolekularni heparin, kako bi spriječio moguće teške komplikacije - kaže naš sugovornik.

Ujedno, u posljednjih godinu dana otkako je COVID sveprisutan, svjetski su ginekolozi došli do jednog zapanjujućeg podatka.

- Primijetili smo da ima dosta više tromboza kod trudnica koje su COVID pozitivne. Nikada nije bilo toliko tromboza. Zbog toga su se na svjetskoj razini donijele smjernice za sprječavanje tromboze kod trudnica. I sve te preporuke idu prema niskomolekularnom heparinu - zaključio je naš sugovornik.
