Imunolog Bojan Polić u Newsroomu N1 komentirao je epidemiološku situaciju.
- Mislim da još nismo dosegnuli vrhunac jer postoji dovoljno osjetljivih ljudi na koje ova delta varijanta može djelovati. Mi imamo samo oko 53% ljudi koji su potpuno cijepljeni, to je još uvijek značajno premalo u odnosu na poželjnih 90% da bi se ova varijanta stavila pod kontrolu, rekao je na početku.
O mjerama
- Naše mjere, ove koje jesu, su dosta blage i one se dobrim dijelom i ne poštuju. Ljudi su se pomalo opustili nakon ljetnog perioda, mi bismo trebali ili postrožiti mjere ili inzistirati na širem korištenju covid potvrda. Time bi postigli dvije stvari, a to je da su ljudi s potvrdama u zatvorenom prostoru bitno sigurniji jer cijepljeni manje prenose virus, a s druge strane ta mjera bi potaknula cijepljenje kao i u drugim zemljama.
Rekao je i kako udio od 30% pozitivnih od svih testiranih govori o tome da je situacija trenutno nekontrolirana i da među nama ima dosta virusa.
- Djeca su u školama, studenti su na fakultetima, oni dosta prenose virusnu infekciju i ako se to ne počne bolje kontrolirati brojke će rasti, a time i broj hospitaliziranih i umrlih, kaže.
Nuspojave kod mladih
Dodaje i kako vjeruje da se treba poticati mlade na cijepljenje:
- Naročito među studentskom i adolescentnom populacijom jer su oni najviše socijalno aktivni i prenose virus mada oni najčešće ne obolijevaju od virusne infekcije. Ali su prenosioci, a kako imamo situaciju da je svega 50-ak posto odraslih cijepljeno, naravno da prenose virus svojim ukućanima, starijim osobama i to rezultira hospitalizacijama i nepotrebnim smrtima.
- Druga stvar je da i mlađi mogu oboljeti, ima pojava i miokarditisa, perikarditisa, inflamatornog sindroma, ne tako često, ali postoji i mlađima korist da se cijepe, zaključuje za N1.
Kaže i kako kod nas postoji veliko nepovjerenje prema cjepivima i općenito prema svim podacima.
“Postoji veliki šum u komunikaciji. Ako pogledate zapadne zemlje, naročito Izrael, oni su rano krenuli i s cijepljenjem djece. To su uglavnom mRNA cjepiva i ona su se pokazala učinkovita i zapravo s vrlo malo nuspojava.
To su uglavnom više iznimke, to su rijetke nuspojave. Kod mladih je primijećeno da u nekom vrlo malom broju slučajeva dolazi do upale srčane ovojnice, no to su blaži oblici, a moramo znati i da virus uzrokuje miokarditis i perikarditis tako da se ipak cijepljenjem smanjuje rizik od toga, objašnjava.
Što s trećom dozom?
- Treća doza, to nije ništa neuobičajeno, naime, imate primjer virusa hepatitisa B, gdje se isto uzimaju tri doze cjepiva. Radi se o booster dozi, svaka doza povrh primarne je booster doza i ona služi tome da se pojača imunosni odogovor i da on dosegne maksimalno moguće veličine i da bi se na taj način mogla spriječiti infekcija ovim prilično zaraznim delta virusom koji je puno infektivniji od prvotnog alfa virusa”, objašnjava.
Dodaje i kako velike kompanije poput Pfizera i Moderne rade na novim formulacijama cjepiva, koje će biti dostupne, vjerojatno, sljedeće godine.
“Zaštita je 90% od težih oblika i smrtnosti”
Objasnio je i imunološki odgovor i koaj je razliak od prirodnog imuniteta i onog stečenog nakon cijepljenja:
- Imunološki odgovor, kao i stanje svih naših organa ovisi o našem općem zdravstvenom stanju. Kada se vi inficirate virusom, dobijete širok dijapazon odgovora imunološkog sustava na različite proteine virusa. Tako da dobijete široki dijapazon odgovora. Kod cijepljena imate samo spike protein koji je dio svih cjepiva koja se sada koriste i imate jako fokusiran imunološki odgovor. Naravno da taj odgovor nije toliko robustan kao preboljenje, no preboljenje nosi veli rizik od teških oblika i smrti, da biste to preveniralik cijepite se.
Što se tiče infekcije cijepljenih, moramo znati da kod 5 do 10 posto cijepljenih s obje doze, ljudi su slabije reagirali na cjepivo. Ništa nije 100% u biologiji, no ako govorimo da je zaštita 90% od težih oblika i smrtnosti, to je visoka razina efikasnosti. Mogu se zaraziti i oni koji su preboljeli jer ako je jako zarazna varijanta, virus se replicira u našim sluznicama, ali to najčešće zavši s vrlo blagim simptomima i brže prolazi, kaže epidemiolog Polić za N1.
Objasnio je i situaciju s antitijelima:
- Visoka razina protutijela ne mora značiti da imamo visoku razinu neutralizacijskih protutijela. Ona nas štite od infekcije. Mi vrlo teško možemo odrediti i stanični imunološki odgovor putem T-limfocita, to nije jednostavno, on je i vrlo individualan. Nije jednostavno odrediti razinu imunološke reaktivnosti na virus. Treća stvar je i što su cjepiva bazirana na Euhanskom soju, a delta ima nešto mutacija. Stoga je vrlo bitno da imamo visoku razinu neutralizacijskih protutijela da nas zaštite od delta soja. Zato se ide na treću dozu da bi smo kompenzirali manjak afiniteta protutijela na delta varijantu.

Imunolog Bojan Polić u Newsroomu N1 komentirao je epidemiološku situaciju.
- Mislim da još nismo dosegnuli vrhunac jer postoji dovoljno osjetljivih ljudi na koje ova delta varijanta može djelovati. Mi imamo samo oko 53% ljudi koji su potpuno cijepljeni, to je još uvijek značajno premalo u odnosu na poželjnih 90% da bi se ova varijanta stavila pod kontrolu, rekao je na početku.
O mjerama
- Naše mjere, ove koje jesu, su dosta blage i one se dobrim dijelom i ne poštuju. Ljudi su se pomalo opustili nakon ljetnog perioda, mi bismo trebali ili postrožiti mjere ili inzistirati na širem korištenju covid potvrda. Time bi postigli dvije stvari, a to je da su ljudi s potvrdama u zatvorenom prostoru bitno sigurniji jer cijepljeni manje prenose virus, a s druge strane ta mjera bi potaknula cijepljenje kao i u drugim zemljama.
Rekao je i kako udio od 30% pozitivnih od svih testiranih govori o tome da je situacija trenutno nekontrolirana i da među nama ima dosta virusa.
- Djeca su u školama, studenti su na fakultetima, oni dosta prenose virusnu infekciju i ako se to ne počne bolje kontrolirati brojke će rasti, a time i broj hospitaliziranih i umrlih, kaže.
Nuspojave kod mladih
Dodaje i kako vjeruje da se treba poticati mlade na cijepljenje:
- Naročito među studentskom i adolescentnom populacijom jer su oni najviše socijalno aktivni i prenose virus mada oni najčešće ne obolijevaju od virusne infekcije. Ali su prenosioci, a kako imamo situaciju da je svega 50-ak posto odraslih cijepljeno, naravno da prenose virus svojim ukućanima, starijim osobama i to rezultira hospitalizacijama i nepotrebnim smrtima.
- Druga stvar je da i mlađi mogu oboljeti, ima pojava i miokarditisa, perikarditisa, inflamatornog sindroma, ne tako često, ali postoji i mlađima korist da se cijepe, zaključuje za N1.
Kaže i kako kod nas postoji veliko nepovjerenje prema cjepivima i općenito prema svim podacima.
“Postoji veliki šum u komunikaciji. Ako pogledate zapadne zemlje, naročito Izrael, oni su rano krenuli i s cijepljenjem djece. To su uglavnom mRNA cjepiva i ona su se pokazala učinkovita i zapravo s vrlo malo nuspojava.
To su uglavnom više iznimke, to su rijetke nuspojave. Kod mladih je primijećeno da u nekom vrlo malom broju slučajeva dolazi do upale srčane ovojnice, no to su blaži oblici, a moramo znati i da virus uzrokuje miokarditis i perikarditis tako da se ipak cijepljenjem smanjuje rizik od toga, objašnjava.
Što s trećom dozom?
- Treća doza, to nije ništa neuobičajeno, naime, imate primjer virusa hepatitisa B, gdje se isto uzimaju tri doze cjepiva. Radi se o booster dozi, svaka doza povrh primarne je booster doza i ona služi tome da se pojača imunosni odogovor i da on dosegne maksimalno moguće veličine i da bi se na taj način mogla spriječiti infekcija ovim prilično zaraznim delta virusom koji je puno infektivniji od prvotnog alfa virusa”, objašnjava.
Dodaje i kako velike kompanije poput Pfizera i Moderne rade na novim formulacijama cjepiva, koje će biti dostupne, vjerojatno, sljedeće godine.
“Zaštita je 90% od težih oblika i smrtnosti”
Objasnio je i imunološki odgovor i koaj je razliak od prirodnog imuniteta i onog stečenog nakon cijepljenja:
- Imunološki odgovor, kao i stanje svih naših organa ovisi o našem općem zdravstvenom stanju. Kada se vi inficirate virusom, dobijete širok dijapazon odgovora imunološkog sustava na različite proteine virusa. Tako da dobijete široki dijapazon odgovora. Kod cijepljena imate samo spike protein koji je dio svih cjepiva koja se sada koriste i imate jako fokusiran imunološki odgovor. Naravno da taj odgovor nije toliko robustan kao preboljenje, no preboljenje nosi veli rizik od teških oblika i smrti, da biste to preveniralik cijepite se.
Što se tiče infekcije cijepljenih, moramo znati da kod 5 do 10 posto cijepljenih s obje doze, ljudi su slabije reagirali na cjepivo. Ništa nije 100% u biologiji, no ako govorimo da je zaštita 90% od težih oblika i smrtnosti, to je visoka razina efikasnosti. Mogu se zaraziti i oni koji su preboljeli jer ako je jako zarazna varijanta, virus se replicira u našim sluznicama, ali to najčešće zavši s vrlo blagim simptomima i brže prolazi, kaže epidemiolog Polić za N1.
Objasnio je i situaciju s antitijelima:
- Visoka razina protutijela ne mora značiti da imamo visoku razinu neutralizacijskih protutijela. Ona nas štite od infekcije. Mi vrlo teško možemo odrediti i stanični imunološki odgovor putem T-limfocita, to nije jednostavno, on je i vrlo individualan. Nije jednostavno odrediti razinu imunološke reaktivnosti na virus. Treća stvar je i što su cjepiva bazirana na Euhanskom soju, a delta ima nešto mutacija. Stoga je vrlo bitno da imamo visoku razinu neutralizacijskih protutijela da nas zaštite od delta soja. Zato se ide na treću dozu da bi smo kompenzirali manjak afiniteta protutijela na delta varijantu.
