Grozimo se dana kada ćemo morati biti egzekutori koji će birati kome spašavati život. Na žalost, ako se popune bolnički kapaciteti, ako ne bude dovoljno respiratora, ako iscrpimo broj ljudi koji na njima znaju raditi, to je scenarij kojega se svi pribojavamo. Vidjeli ste što se događalo u Italiji – mračne su to prognoze vezane za širenje zaraze koronavirusom u Hrvatskoj i izdržljivost zdravstvenog sustava na koje upozorava dr. Ivana Šmit, predsjednica Hrvatske udruge bolničkih liječnika (HUBOL) i dopredsjednica Hrvatske liječničke komore.

Stanje u bolnicama je pred kolapsom, zdravstveni djelatnici padaju s nogu, bolnički se kapaciteti popunjavaju zadnjih dana velikom brzinom, iz bolnica diljem zemlje stižu dramatični apeli zdravstvenog osoblja. A znanstvenici, među njima i prof. dr. Ozren Polašek, pročelnik Katedre za javno zdravstvo i voditelj Centra za globalno zdravlje Medicinskog fakulteta Sveučilišta u Splitu te član Vladina znanstvenog savjeta, upozoravaju kako još nismo na vrhuncu epidemije.

Brojke hospitaliziranih i umrlih od COVID-a 19 penju se iz dana u dan: na isti dan prije točno mjesec dana na bolničkom smo liječenju imali 289 pacijenata, 23 pacijenta na respiratoru, a dvije osobe su bile umrle. U utorak 3. studenog, Nacionalni stožer civilne zaštite izvijestio je kako su u posljednja 24 sata od novog koronavirusa preminule 34 osobe, a trenutačno je na bolničkom liječenju 1191 osoba (64 više nego prethodni dan), dok je na respiratoru 112 pacijenata ili 18 više nego dan prije.

Sve više preminulih

- Situacija je iznimno loša, bolnice su prepunjene. To je nešto što ne vjeruje onaj tko ne vidi. Nažalost, svjedočimo i jednom organizacijskom fijasku – ističe dr. Ivana Šmit, dodajući kako je teško i nezahvalno prognozirati kada će sustav totalno "popucati" i kada više nećemo unutar postojećih bolničkih kapaciteta moći zbrinjavati oboljele.

- Teško je to reći. Može biti za pet, deset, 15 dana ako se nastavi ovakvo punjenje bolničkih kapaciteta. Broj pozitivnih ovisit će o broju dnevno testiranih. Dosad znamo kako oko deset posto zaraženih zahtijeva bolničko liječenje, a određeni broj završi na respiratoru – ističe predsjednica HUBOL-a.

Hospitalizacija i oporavak osoba koje zbog COVID-a 19 završe na respiratoru su dugotrajni, a dr. Šmit dodaje kako samo terapijska ventilacija traje tjednima, barem tri, četiri tjedna. Nažalost, statistika je tu neumoljiva, smrtnost osoba na respiratoru je visokih 50 posto.

Hospitalno liječenje osoba oboljelih od COVID-a 19, a koje ne zahtijeva priključenje na respirator, za osobe srednje dobi od 40, 50 godina, a koje recimo dobiju upalu pluća, može trajati od osam do deset dana, a onih starije dobi još i dulje, do 15-ak dana. To, naravno, ovisi o kliničkoj slici i postojanju komorbiditeta, odnosno drugih bolesti. Jasno je onda koliko je bolnički sustav opterećen, a pristižu novi bolesnici. S druge strane, usmjerenost na borbu protiv COVID-a 19 po strani je stavila druge bolesti, pa se dr. Šmit pribojava kako će deficit u zbrinjavanju osoba s ostalim teškim bolestima, od kardiovaskularnih do onkoloških, eskalirati za šest mjeseci do godine dana.

'Stanje je neodrživo'

Što tek reći o nedostatku kadra?

- Inače nam nedostaje oko dvije do dvije i po tisuće liječnika da bi sustav normalno funkcionirao. Možete misliti kako je sada kada dio zdravstvenih djelatnika završi u samoizolaciji ili oboli od COVID-a 19. Stanje je neodrživo i politika mora povući pravi potez kako bi se zaustavili ovi kuljajući procesi u zdravstvu – poručuje dr. Ivana Šmit. HUBOL je zajedno s Hrvatskom liječničkom komorom, Hrvatskim liječničkim sindikatom, Koordinacijom hrvatske obiteljske medicine (KoHOM) te infektološkim i mikrobiološkim društvom Hrvatskog liječničkog zbora uputio zahtjev Vladi za postroženjem epidemioloških mjera.

- Apeliramo na politiku da postupi poput zemalja u okruženju i obuzda ponašanja koja pridonose širenju zaraze i daljnjem opterećenju zdravstvenog sustava. Vidimo da se dio ljudi još uvijek ne želi odreći vlastitiog komoditeta. Lockdown? Neki oblik da. U ovom trenutku treba reducirati sve aktivnosti koje nisu od vitalnog značenja, od rada kafića, restorana do ograničavanja broja ljudi na radnome mjestu i omogućavanja rada od kuće, kao i redukcije kretanja i okupljanja ljudi u velikim skupinama. Nošenje maski treba biti obvezno u javnom transportu, u svim zatvorenim prostorima, a nenošenje maski treba penalizirati – navodi dr. Šmit neke od mjera koje traže od mjerodavnih.

- Prema mojim procjenama, stanje je dramatično i samo što nije puknulo do kraja. Ako premijer kaže kako nema razloga za paniku, onda on nema realan uvid u stvarnost u bolnicama. Bolnički sustav ne može još dugo, ne samo kada je riječ o bolničkim kapacitetima, nego i po pitanju opreme i kadra, kako liječnika, tako i medicinskih sestara – odlučno tvrdi dr. Renata Čulinović Čaić, predsjednica Hrvatskog liječničkog sindikata.

Grozimo se dana kada ćemo morati biti egzekutori koji će birati kome spašavati život. Na žalost, ako se popune bolnički kapaciteti, ako ne bude dovoljno respiratora, ako iscrpimo broj ljudi koji na njima znaju raditi, to je scenarij kojega se svi pribojavamo. Vidjeli ste što se događalo u Italiji – mračne su to prognoze vezane za širenje zaraze koronavirusom u Hrvatskoj i izdržljivost zdravstvenog sustava na koje upozorava dr. Ivana Šmit, predsjednica Hrvatske udruge bolničkih liječnika (HUBOL) i dopredsjednica Hrvatske liječničke komore.

Stanje u bolnicama je pred kolapsom, zdravstveni djelatnici padaju s nogu, bolnički se kapaciteti popunjavaju zadnjih dana velikom brzinom, iz bolnica diljem zemlje stižu dramatični apeli zdravstvenog osoblja. A znanstvenici, među njima i prof. dr. Ozren Polašek, pročelnik Katedre za javno zdravstvo i voditelj Centra za globalno zdravlje Medicinskog fakulteta Sveučilišta u Splitu te član Vladina znanstvenog savjeta, upozoravaju kako još nismo na vrhuncu epidemije.

Brojke hospitaliziranih i umrlih od COVID-a 19 penju se iz dana u dan: na isti dan prije točno mjesec dana na bolničkom smo liječenju imali 289 pacijenata, 23 pacijenta na respiratoru, a dvije osobe su bile umrle. U utorak 3. studenog, Nacionalni stožer civilne zaštite izvijestio je kako su u posljednja 24 sata od novog koronavirusa preminule 34 osobe, a trenutačno je na bolničkom liječenju 1191 osoba (64 više nego prethodni dan), dok je na respiratoru 112 pacijenata ili 18 više nego dan prije.

Sve više preminulih

- Situacija je iznimno loša, bolnice su prepunjene. To je nešto što ne vjeruje onaj tko ne vidi. Nažalost, svjedočimo i jednom organizacijskom fijasku – ističe dr. Ivana Šmit, dodajući kako je teško i nezahvalno prognozirati kada će sustav totalno "popucati" i kada više nećemo unutar postojećih bolničkih kapaciteta moći zbrinjavati oboljele.

- Teško je to reći. Može biti za pet, deset, 15 dana ako se nastavi ovakvo punjenje bolničkih kapaciteta. Broj pozitivnih ovisit će o broju dnevno testiranih. Dosad znamo kako oko deset posto zaraženih zahtijeva bolničko liječenje, a određeni broj završi na respiratoru – ističe predsjednica HUBOL-a.

Hospitalizacija i oporavak osoba koje zbog COVID-a 19 završe na respiratoru su dugotrajni, a dr. Šmit dodaje kako samo terapijska ventilacija traje tjednima, barem tri, četiri tjedna. Nažalost, statistika je tu neumoljiva, smrtnost osoba na respiratoru je visokih 50 posto.

Hospitalno liječenje osoba oboljelih od COVID-a 19, a koje ne zahtijeva priključenje na respirator, za osobe srednje dobi od 40, 50 godina, a koje recimo dobiju upalu pluća, može trajati od osam do deset dana, a onih starije dobi još i dulje, do 15-ak dana. To, naravno, ovisi o kliničkoj slici i postojanju komorbiditeta, odnosno drugih bolesti. Jasno je onda koliko je bolnički sustav opterećen, a pristižu novi bolesnici. S druge strane, usmjerenost na borbu protiv COVID-a 19 po strani je stavila druge bolesti, pa se dr. Šmit pribojava kako će deficit u zbrinjavanju osoba s ostalim teškim bolestima, od kardiovaskularnih do onkoloških, eskalirati za šest mjeseci do godine dana.

'Stanje je neodrživo'

Što tek reći o nedostatku kadra?

- Inače nam nedostaje oko dvije do dvije i po tisuće liječnika da bi sustav normalno funkcionirao. Možete misliti kako je sada kada dio zdravstvenih djelatnika završi u samoizolaciji ili oboli od COVID-a 19. Stanje je neodrživo i politika mora povući pravi potez kako bi se zaustavili ovi kuljajući procesi u zdravstvu – poručuje dr. Ivana Šmit. HUBOL je zajedno s Hrvatskom liječničkom komorom, Hrvatskim liječničkim sindikatom, Koordinacijom hrvatske obiteljske medicine (KoHOM) te infektološkim i mikrobiološkim društvom Hrvatskog liječničkog zbora uputio zahtjev Vladi za postroženjem epidemioloških mjera.

- Apeliramo na politiku da postupi poput zemalja u okruženju i obuzda ponašanja koja pridonose širenju zaraze i daljnjem opterećenju zdravstvenog sustava. Vidimo da se dio ljudi još uvijek ne želi odreći vlastitiog komoditeta. Lockdown? Neki oblik da. U ovom trenutku treba reducirati sve aktivnosti koje nisu od vitalnog značenja, od rada kafića, restorana do ograničavanja broja ljudi na radnome mjestu i omogućavanja rada od kuće, kao i redukcije kretanja i okupljanja ljudi u velikim skupinama. Nošenje maski treba biti obvezno u javnom transportu, u svim zatvorenim prostorima, a nenošenje maski treba penalizirati – navodi dr. Šmit neke od mjera koje traže od mjerodavnih.

- Prema mojim procjenama, stanje je dramatično i samo što nije puknulo do kraja. Ako premijer kaže kako nema razloga za paniku, onda on nema realan uvid u stvarnost u bolnicama. Bolnički sustav ne može još dugo, ne samo kada je riječ o bolničkim kapacitetima, nego i po pitanju opreme i kadra, kako liječnika, tako i medicinskih sestara – odlučno tvrdi dr. Renata Čulinović Čaić, predsjednica Hrvatskog liječničkog sindikata.
