Smrt petogodišnjeg dječaka koji je danas pronađen u zatvorenom automobilu u Kninu šokirala je javnost. Na žalost ovakve vijesti zateknu nas svake godine...

Pretpostavka je da je dječak doživio hipertermiju.

- Tijelo održava tjelesnu temperaturu u uskom temperaturnom rasponu zahvaljujući ravnoteži stvaranja ili primanja te otpuštanja topline. Do hipertermije dolazi kada se središnja tjelesna temperatura povisi iznad fizioloških vrijednosti, a kao posljedica zatajenja mehanizama termoregulacije. Zatajenje nastaje zbog značajnog rasta okolišne temperature ili zbog nemogućnosti tijela da se oslobodi od endogeno nastale topline. Naravno, moguće je da uzrok bude kombiniran, primjerice zbog ostavljanja djeteta u automobilu na suncu te nakon dužeg vježbanja ili boravka u vrućem okružju, prof. dr. sc. Hipertermija je uzrok znatnoga morbiditeta, posebice male djece o čemu čujemo svake godine i u svim dijelovima svijeta. Ona nažalost nerijetko i umiru ostavljena u loše ventiliranim prostorijama ili u zaključanim automobilima pri vanjskim temperaturama 30 – 40 °C. Naime, manja djeca imaju razmjerno veću površinu tijela s obzirom na svoju tjelesnu masu te pojačano apsorbiraju okolišnu toplinu u temperaturno zahtjevnim uvjetima, pojašnjava dr. Markić.

Hipertermija je u adolescenata često vezana s intenzivnim treningom i vježbanjem jer stvaraju veću količinu topline po kilogramu tjelesne težine, počinju se znojiti pri višim tjelesnim temperaturama u usporedbi s odraslima, a znaju i slabije uzimati napitke tijekom vježbanja.

Porast tjelesne temperature povezan je s povećanim iskorištavanjem kisika i pojačanom metaboličkom aktivnosti koji rezultiraju ubrzanim disanjem i bržim srčanim ritmom. Pri tjelesnoj temperaturi višoj od 42 °C dolazi do sloma mnogih metaboličkih procesa te multiorganskog zatajenja.

Kako izgleda klinička slika hipertermije?

- Klinička slika hipertermije je različita i može biti u rasponu od blažih simptoma poput tople i zacrvenjene kože, povraćanja, gubitka svijesti, mišićnih grčeva do životno ugrožavajućih stanja toplinske iscrpljenosti ili udara. Toplinski udar se očituje neprimjerenim ponašanjem ili rasuđivanjem, halucinacijama, nesigurnim hodom i govorom, ugroženošću dišnog sustava i disanja te cirkulacije, a u najtežim slučajevima vodi do kome i smrti.

Što možete savjetovati roditeljima ako im dijete doživi hipertermiju?

- U izvanbolničkim uvjetima se stanja hipertermije liječi primjerenim oralnim unosom tekućine s dodatkom elektrolita. Prije toga dijete treba svući i postaviti u hladniji okoliš. Hlađenje se može provoditi sprejanjem ili prskanjem bolesnika mlakom vodom, potapanjem u vodu ili oblozima. Ako je bolesnik teže dehidriran ili ima sniženi krvni tlak, treba mu dati infuziju kristaloida. Ako simptomi postoje i nakon intravenske i peroralne nadoknade elektrolita i tekućine, liječenje treba nastaviti u bolničkim uvjetima, pojašnjava dr. Markić.

- S obzirom na iskustvo koje imamo osobito u ljetnim mjesecima apeliram na odgovorno ponašanje, izbjegavanje dugog boravka na vrućini, primjerenu hidraciju i uzimanje izotoničkih napitaka uz strogo izbjegavanje energetskih pripravaka. također vrlo je važno nositi laganu pamučnu odjeću - savjetuje dr. Markić.

Smrt petogodišnjeg dječaka koji je danas pronađen u zatvorenom automobilu u Kninu šokirala je javnost. Na žalost ovakve vijesti zateknu nas svake godine...

Pretpostavka je da je dječak doživio hipertermiju.

- Tijelo održava tjelesnu temperaturu u uskom temperaturnom rasponu zahvaljujući ravnoteži stvaranja ili primanja te otpuštanja topline. Do hipertermije dolazi kada se središnja tjelesna temperatura povisi iznad fizioloških vrijednosti, a kao posljedica zatajenja mehanizama termoregulacije. Zatajenje nastaje zbog značajnog rasta okolišne temperature ili zbog nemogućnosti tijela da se oslobodi od endogeno nastale topline. Naravno, moguće je da uzrok bude kombiniran, primjerice zbog ostavljanja djeteta u automobilu na suncu te nakon dužeg vježbanja ili boravka u vrućem okružju, prof. dr. sc. Hipertermija je uzrok znatnoga morbiditeta, posebice male djece o čemu čujemo svake godine i u svim dijelovima svijeta. Ona nažalost nerijetko i umiru ostavljena u loše ventiliranim prostorijama ili u zaključanim automobilima pri vanjskim temperaturama 30 – 40 °C. Naime, manja djeca imaju razmjerno veću površinu tijela s obzirom na svoju tjelesnu masu te pojačano apsorbiraju okolišnu toplinu u temperaturno zahtjevnim uvjetima, pojašnjava dr. Markić.

Hipertermija je u adolescenata često vezana s intenzivnim treningom i vježbanjem jer stvaraju veću količinu topline po kilogramu tjelesne težine, počinju se znojiti pri višim tjelesnim temperaturama u usporedbi s odraslima, a znaju i slabije uzimati napitke tijekom vježbanja.

Porast tjelesne temperature povezan je s povećanim iskorištavanjem kisika i pojačanom metaboličkom aktivnosti koji rezultiraju ubrzanim disanjem i bržim srčanim ritmom. Pri tjelesnoj temperaturi višoj od 42 °C dolazi do sloma mnogih metaboličkih procesa te multiorganskog zatajenja.

Kako izgleda klinička slika hipertermije?

- Klinička slika hipertermije je različita i može biti u rasponu od blažih simptoma poput tople i zacrvenjene kože, povraćanja, gubitka svijesti, mišićnih grčeva do životno ugrožavajućih stanja toplinske iscrpljenosti ili udara. Toplinski udar se očituje neprimjerenim ponašanjem ili rasuđivanjem, halucinacijama, nesigurnim hodom i govorom, ugroženošću dišnog sustava i disanja te cirkulacije, a u najtežim slučajevima vodi do kome i smrti.

Što možete savjetovati roditeljima ako im dijete doživi hipertermiju?

- U izvanbolničkim uvjetima se stanja hipertermije liječi primjerenim oralnim unosom tekućine s dodatkom elektrolita. Prije toga dijete treba svući i postaviti u hladniji okoliš. Hlađenje se može provoditi sprejanjem ili prskanjem bolesnika mlakom vodom, potapanjem u vodu ili oblozima. Ako je bolesnik teže dehidriran ili ima sniženi krvni tlak, treba mu dati infuziju kristaloida. Ako simptomi postoje i nakon intravenske i peroralne nadoknade elektrolita i tekućine, liječenje treba nastaviti u bolničkim uvjetima, pojašnjava dr. Markić.

- S obzirom na iskustvo koje imamo osobito u ljetnim mjesecima apeliram na odgovorno ponašanje, izbjegavanje dugog boravka na vrućini, primjerenu hidraciju i uzimanje izotoničkih napitaka uz strogo izbjegavanje energetskih pripravaka. također vrlo je važno nositi laganu pamučnu odjeću - savjetuje dr. Markić.

Smrt petogodišnjeg dječaka koji je danas pronađen u zatvorenom automobilu u Kninu šokirala je javnost. Na žalost ovakve vijesti zateknu nas svake godine...

Pretpostavka je da je dječak doživio hipertermiju.

- Tijelo održava tjelesnu temperaturu u uskom temperaturnom rasponu zahvaljujući ravnoteži stvaranja ili primanja te otpuštanja topline. Do hipertermije dolazi kada se središnja tjelesna temperatura povisi iznad fizioloških vrijednosti, a kao posljedica zatajenja mehanizama termoregulacije.
