Afte su stvarno neugodna ranica koja se ukaže na jeziku, usnama ili desnima, a znamo da je tu jer osjećamo žareću bol. Nisu zarazne, ali se rado ponavljaju i obično nas muče između tjedan i deset dana dok se konačno ne povuku. U borbi s njima koristi se cijeli spektar lijekova i prirodnih preparata, ali zašto nastaju i kako im medicina može doskočiti otkrila nam je doc. dr sc. Marisa Klančnik, otorinolaringolog-audiolog na Klinici za bolesti uha, grla i nosa s kirurgijom glave i vrata KBC-a Split.
- Radi se o načešćoj i najrasprostranjenijoj bolesti usne šuljine i češća je u mlađe populacije, naročito u djece, djevojaka i žena te osoba izloženih pojačanom stresu, a često govorimo i o obiteljskoj predispoziciji. Procjenjuje se da bolest pogađa oko 20 posto svjetske populacije. Prema mehanizmu nastanka može se svrstati u autoimune bolesti zbog stvaranja antitijela prema vlastitim stanicama epitela usne šupljine.
Što uzrokuje afte i jesu li im neki ljudi skloniji?
- Etiologija afti je multifaktorijalna - uzrok može biti alergija, stres, trauma poput ozljede zubnom četkicom, protezom, ugrizom usta ili jezika, vrućom hranom, hormonalne promjene, pušenje, imunološki poremećaji, gastrointestinalne bolesti, nutricionalni deficiti, anemija. Budući da se često ponavljaju može se govoriti o sklonosti za nastanak afti. U određenim slučajevima afte se mogu pojaviti kod prisustva bakterije Helicobacter Pylori. Manjak željeza, vitamina B12 i folne kiseline nađen je u oko 20 posto oboljelih.
Ranice se otvaraju u usnoj šupljini, ali simptomi mogu biti od relativno blagih do iznimno neugodnih...
- Najčešće lokacije su obraz, jezik i predvorje usne šupljine, a glavni simptomi bolesti su peckanje, žarenje i intenzivna bol. Pojavljuju se u obliku uglavnom manjih bolnih ranica na sluznici usne šupljine. Afte često ometaju pacijenta u svakodnevnim aktivnostima i funkcijama poput žvakanja, gutanja i govora. Teži oblici afti mogu dovesti do povišene temperature, lošeg općeg stanja organizma i povećanja i bolnosti lokalnih limfnih čvorova. Inače, afte se javljaju u tri glavne forme: minor forma (male afte); najčešći su tip, promjene 3-10 mm veličine i zacijeljuju u dva tjedna bez ožiljaka. Pojavljuju se na sluznici obraza i usana, često ih ima nekoliko u skupinama, a boja im je blijedožuta ili bijela s crvenim rubovima. major forma (velike afte), promjene su preko jednog cm veličine, a cijeljenje traje do šest tjedana s mogućim ožiljkom. Češće se pojavljuju pojedinačno. Treći je herpetiformni tip - afte koje nalikuju herpesu, višestruke su i najviše prisutne na sluznici jezika, podjezičnom području i obrazu.
Kako se najbrže mogu izliječiti?
- Kod liječenja u prvom redu treba isključiti neku od sistemskih bolesti poput dijabetesa, bolesti štitnjače, Chronove bolesti i ulceroznog kolitisa, gastroezofagealnog refluksa, sideropenične/perniciozne anemije, bubrežnih bolesti, adrenalne insuficijencije (insuficijencije nadbubrežne žlijezde), povišenog bilirubina i Pemfigusa vulgarisa.
Nakon toga potrebno je ublažiti bolove primjenom lokalnih ili sistemskih analgetika te ispiranjem usne šupljine različitim antiseptičkim otopinama, a uporabom lokalnih kortikosteroida u lakšim slučajevima te sistemskih kortikosteroida u težim slučajevima, smirujemo upalu i omogućavamo bolje cijeljenje. Često se primjenjuju i gelovi na bazi hijaluronske kiseline i aloe vera koje pomažu prirodnom cijeljenju oštećenog tkiva. Zdravom prehranom, izbjegavanjem stresa i primjenom vitamina djelujemo na podizanje imuniteta organizma. U slučajevima manjka željeza, vitamina B12 ili folne kiseline, preporučuje se njihova nadoknada.
A ako ništa od toga ne pomaže?
- Ukoliko dođe do povećanja afte ili ne dođe do cijeljenja u roku od 14 dana, ako se pojavi kašalj ili povišena tempetarura, ako perzistiraju povećani regionalni limfni čvorovi, ako je zahvaćeno zubno meso-gingiva, ako postoji sklonost čestim infekcijama, ako se afte pojavljuju vrlo često ili se sumnja na pojavnost afte kao posljedice uzimanja nekog lijeka - obavezno treba potražiti liječničku pomoć. Ipak, važno je naglasiti da afte nisu zarazna bolest i ne mogu se prenijeti drugoj osobi, što je svakako dobra vijest, zaključuje naša sugovornica i dodaje da u podlozi ove bezopasne ali izrazito bolne pojave leži narušeni imunitet.

Afte su stvarno neugodna ranica koja se ukaže na jeziku, usnama ili desnima, a znamo da je tu jer osjećamo žareću bol. Nisu zarazne, ali se rado ponavljaju i obično nas muče između tjedan i deset dana dok se konačno ne povuku. U borbi s njima koristi se cijeli spektar lijekova i prirodnih preparata, ali zašto nastaju i kako im medicina može doskočiti otkrila nam je doc. dr sc. Marisa Klančnik, otorinolaringolog-audiolog na Klinici za bolesti uha, grla i nosa s kirurgijom glave i vrata KBC-a Split.
- Radi se o načešćoj i najrasprostranjenijoj bolesti usne šuljine i češća je u mlađe populacije, naročito u djece, djevojaka i žena te osoba izloženih pojačanom stresu, a često govorimo i o obiteljskoj predispoziciji. Procjenjuje se da bolest pogađa oko 20 posto svjetske populacije. Prema mehanizmu nastanka može se svrstati u autoimune bolesti zbog stvaranja antitijela prema vlastitim stanicama epitela usne šupljine.
Što uzrokuje afte i jesu li im neki ljudi skloniji?
- Etiologija afti je multifaktorijalna - uzrok može biti alergija, stres, trauma poput ozljede zubnom četkicom, protezom, ugrizom usta ili jezika, vrućom hranom, hormonalne promjene, pušenje, imunološki poremećaji, gastrointestinalne bolesti, nutricionalni deficiti, anemija. Budući da se često ponavljaju može se govoriti o sklonosti za nastanak afti. U određenim slučajevima afte se mogu pojaviti kod prisustva bakterije Helicobacter Pylori. Manjak željeza, vitamina B12 i folne kiseline nađen je u oko 20 posto oboljelih.
Ranice se otvaraju u usnoj šupljini, ali simptomi mogu biti od relativno blagih do iznimno neugodnih...
- Najčešće lokacije su obraz, jezik i predvorje usne šupljine, a glavni simptomi bolesti su peckanje, žarenje i intenzivna bol. Pojavljuju se u obliku uglavnom manjih bolnih ranica na sluznici usne šupljine. Afte često ometaju pacijenta u svakodnevnim aktivnostima i funkcijama poput žvakanja, gutanja i govora. Teži oblici afti mogu dovesti do povišene temperature, lošeg općeg stanja organizma i povećanja i bolnosti lokalnih limfnih čvorova. Inače, afte se javljaju u tri glavne forme: minor forma (male afte); najčešći su tip, promjene 3-10 mm veličine i zacijeljuju u dva tjedna bez ožiljaka. Pojavljuju se na sluznici obraza i usana, često ih ima nekoliko u skupinama, a boja im je blijedožuta ili bijela s crvenim rubovima. major forma (velike afte), promjene su preko jednog cm veličine, a cijeljenje traje do šest tjedana s mogućim ožiljkom. Češće se pojavljuju pojedinačno. Treći je herpetiformni tip - afte koje nalikuju herpesu, višestruke su i najviše prisutne na sluznici jezika, podjezičnom području i obrazu.
Kako se najbrže mogu izliječiti?
- Kod liječenja u prvom redu treba isključiti neku od sistemskih bolesti poput dijabetesa, bolesti štitnjače, Chronove bolesti i ulceroznog kolitisa, gastroezofagealnog refluksa, sideropenične/perniciozne anemije, bubrežnih bolesti, adrenalne insuficijencije (insuficijencije nadbubrežne žlijezde), povišenog bilirubina i Pemfigusa vulgarisa.
Nakon toga potrebno je ublažiti bolove primjenom lokalnih ili sistemskih analgetika te ispiranjem usne šupljine različitim antiseptičkim otopinama, a uporabom lokalnih kortikosteroida u lakšim slučajevima te sistemskih kortikosteroida u težim slučajevima, smirujemo upalu i omogućavamo bolje cijeljenje. Često se primjenjuju i gelovi na bazi hijaluronske kiseline i aloe vera koje pomažu prirodnom cijeljenju oštećenog tkiva. Zdravom prehranom, izbjegavanjem stresa i primjenom vitamina djelujemo na podizanje imuniteta organizma. U slučajevima manjka željeza, vitamina B12 ili folne kiseline, preporučuje se njihova nadoknada.
A ako ništa od toga ne pomaže?
- Ukoliko dođe do povećanja afte ili ne dođe do cijeljenja u roku od 14 dana, ako se pojavi kašalj ili povišena tempetarura, ako perzistiraju povećani regionalni limfni čvorovi, ako je zahvaćeno zubno meso-gingiva, ako postoji sklonost čestim infekcijama, ako se afte pojavljuju vrlo često ili se sumnja na pojavnost afte kao posljedice uzimanja nekog lijeka - obavezno treba potražiti liječničku pomoć. Ipak, važno je naglasiti da afte nisu zarazna bolest i ne mogu se prenijeti drugoj osobi, što je svakako dobra vijest, zaključuje naša sugovornica i dodaje da u podlozi ove bezopasne ali izrazito bolne pojave leži narušeni imunitet.
